Urban dictionaryで“old bag of bones”を調べると“A skinny old person. Usually used as a derogatory term.”と表記されており、「やせ型の人」を表すことが分かりました。

ちなみにbagを『ジーニアス英和辞典』（第五版、大修館書店）でひくと「魅力のない老女、不機嫌な老女」とありました。
