ちょっとしたおふざけが、とんでもない事件へと発展してしまいました。
Junior high student killed by lance lodged in the head
A junior high school student was killed Saturday after being hit in the head with a lance while swimming in a river in Nara Prefecture, police said.
The boy’s younger brother, also a junior high school student and a resident of Nara, was arrested on suspicion of throwing the lance at the boy and injuring him, they said.
“I threw the lance to give it to a friend but accidentally hit my brother,” who was crouching in the river, the victim’s brother was quoted as telling investigators.
The accident occurred in a river in the village of Higashiyoshino in the afternoon while the brothers were swimming there. The lance that hit the victim was about 2 meters long and was made of bamboo and tipped with a metal blade.

また『新英和大辞典』（第六版，研究社）には、「〈弾丸など〉撃ち込む，〈矢を〉突き立てる，〈魚の骨を〉ひっかける[in]」と載っておりました。魚の骨にも使えるところに面白みを感じました。
これらをふまえ見出し部は、「頭に槍が刺さり中学生死亡」のような訳になるかと思います。
