これで今年2度目の入院となった明仁天皇陛下。上記の赤字部分ではその理由として考えられることが挙げられています。
constitutionalは「憲法の，憲法上の」という意味です（『ジーニアス英和辞典』第4版，大修館書店）。ここでは宮内庁の話なので，皇室典範のことをさしていると思います。the Imperial Household Agencyは「宮内庁」という意味です。これらを踏まえて上記の赤字部分を訳してみると，
「（明仁天皇陛下の健康に影響を与えているストレスとして）皇太子徳仁親王の男系後継者不在問題や女系天皇、皇位継承順位繰り上げのために皇室典範の改正を巡る度重なる議論が原因と考えられている。」となると思います。（GP）
